Survey for parents of students with Autism Spectrum Disorder
The Limestone District School Board invites parents' feedback and input regarding the implementation of Policy/Program Memorandum 140, Incorporating Methods of Applied Behaviour Analysis (ABA) into Programs for Students with Autism Spectrum Disorders (ASD).
Parents can access the survey on the LDSB home page.
The survey is open until the end of May 2015.
PPM 140 was released May 17, 2007 and provides direction to school boards to support their use of applied behaviour analysis (ABA) as an effective instructional approach in the education of many students with ASD.
PPM 140 includes a commitment to monitor school boards’ implementation of PPM 140 including the completion of an annual survey by all elementary and secondary schools, with results shared with the Ministry of Education.
In addition to addressing this commitment in PPM 140, the Limestone District School Board wishes to share the survey with parents and guardians of students with ASD and ask them to complete it. Please note that the Ministry will not be collecting the results of parent-completed surveys. However, the data collected by the Limestone District School Board will be used internally for improvement planning and professional learning processes.

Proof of your child's age (birth certificate, baptismal certificate or passport)
Proof of address (driver's licence of parent/guardian, household bills, federal government forms)
Proof of immunization (child's immunization record - visit KFL&A Public Health for more information)
Your child's Ontario Health Card (optional)
We are currently accepting registrations for junior and senior kindergarten students for the 2015-2016 school year, for those children who will turn 4 or 5 by December 31, 2015. Families may register their children in person at the school. When you come in to register, please bring your child's Birth Certificate, Health Card, Immunization Card, and proof of home address.
